Nap Nanny Donation Program Application

Requirements:
The Nap Nanny must be going to a baby with LM, TM or BM.
The baby in need must be under 18 months of age.
The baby in need must live within the USA or Canada unless the location is cleared by CWL prior to receiving payment.
Details:
There is a $30.00 (for USA) and $35.00 (to Canada) flat rate s&h cost that can be paid via PayPal or check. Once the s&h balance is paid the Nap Nanny will be shipped out, within 1-2 business days. The Nap Nanny is shipped USPS Parcel Post with a tracking number and should reach it's destination/customs within 5-10 business days. When shipping Nap Nanny's out of the USA the receiver may be subject to custom charges. CWL is NOT responsible for these charges.
Note 1: DO NOT make payment until you have received an Email from CWL stating your child was approved for this program!
Note 2: Local pick up in available- Port Monmouth, NJ
The Nap Nanny will be held for two weeks after receiving a completed application. There is also a two week grace period for payment. If CWL does not receive payment or any form of communication within those two weeks CWL holds the right to pass the Nap Nanny to another baby in need.

Needed Documentation:
Proof of address (example: copy of drivers license, utility bill, ect)
Proof of child's age
Written letter (on letter head) from child's ENT stating the date of and full diagnosis of your child along with a summary of the current treatment plan.
